I am available and accept the
opportunity to represent New Zealand in the
(name of event)
for which I have been selected by Petanque
New Zealand (PNZ).

I agree to pay PNZ a deposit
of $ (amount)
by (date). I
accept that the deposit will only be
refunded if sufficient funds are raised to
cover the costs of the team participating in
the (event).

I accept that to the extent
that PNZ fundraising is not successful, I
will be required to pay the cost of my
travel, accommodation and any additional
uniform items over and above those supplied
by PNZ, estimated to be
($ amount).

I am bound by the
constitution and bylaws of Petanque New
Zealand (PNZ).

I will accept all decisions
of PNZ in regard to my selection and
representing New Zealand.

I acknowledge that team
selection at the
(event) will rest with the management
team for the event consisting of
(Coach,
Manager/Team Representative).(as applicable)

I accept that the appointment
of the Coach will be at the sole discretion
of PNZ.

I accept that the Team
Representative will be nominated by the team
members, but that the final appointment will
be at the discretion of PNZ.

I agree to be bound by all
directions of PNZ on travel arrangements and
conditions, dress code, behaviour code and
all other directions relating to the event.

I acknowledge that I will
train/practice as required for the
(event) to the
best of my ability and under the responsible
direction of PNZ.

I acknowledge that should I
fail to observe any of the requirements of
PNZ that I may be the subject of a ‘show
cause’ notice from PNZ as to why I should
not be disciplined in accordance with the
rules of the game or the constitution and
bylaws of PNZ and such discipline may
include suspension and/or removal from the
New Zealand team.
